Abstract:

Training in internal medicine nursing involves more than learning a sequence of procedures. Students also have to observe the patient and decide what to do when the condition changes. In this teaching design, cases of chronic obstructive pulmonary disease, heart failure, diabetes, and upper gastrointestinal bleeding are used throughout four linked stages: virtual rehearsal before class, practice with equipment in class, management of a changing scenario, and feedback after class. The virtual case supplies clinical data and asks students to make nursing choices. The skills laboratory is used for the parts that require actual equipment, correct technique, communication, and cooperation. Instead of presenting all information at once, the teacher adds new findings during the exercise. Students then reassess the patient and, when necessary, change their plan. Repeating this cycle places separate skills within a complete nursing task and makes the link between assessment, action, and reassessment easier to see. The model is intended as a workable way to use digital simulation in an internal medicine nursing course.
