摘要(Abstract)：

小细胞肺癌（smallcelllungcancer，SCLC）是一种高度恶性的神经内分泌肿瘤，具有生长迅速、早期转移、初治敏感但易复发和耐药等特点。近年来，含铂化疗联合免疫治疗已成为广泛期SCLC的一线标准方案，但多数患者仍在较短时间内出现疾病进展，进入复发或难治阶段后治疗选择有限，预后较差。传统后线治疗主要包括拓扑替康、CAV方案及铂类再挑战，但总体缓解率和生存获益均不理想。随着对SCLC分子生物学特征认识的加深，卢比替定、DLL3靶向治疗、DNA损伤修复相关靶向治疗以及免疫联合策略不断取得进展。同时，基于ASCL1、NEUROD1、POU2F3及炎症相关亚型的分子分型体系逐渐完善，SLFN11、DLL3、ctDNA等生物标志物也为精准治疗提供了新的依据。本文就复发及难治性SCLC的治疗现状、新进展及精准治疗方向作一综述。
