摘要(Abstract):
目的本研究利用全球疾病负担数据,分析全球与中国老年人轻度创伤性脑损伤(mTBI)的流行趋势,为制定科学的预防策略和减轻疾病负担提供依据。方法采用GBD2021数据,分析全球及中国60岁以上老年人mTBI的年龄标化发病率、患病率、伤残损失寿命年(YLD),计算估计年百分比变化(EAPC)探究疾病负担趋势、发病风险因素,并预测中国60岁以上老年人2022-2040年mTBI 的疾病负担变化趋势。结果2021年全球mTBI疾病负担仍较严峻,患病、发病及YLD人数较1990年增多,但年龄标化指标均呈下降趋势。中国老年人mTBI负担增幅更明显,各项年龄标化率均逐年上升。老年男性各项负担指标始终高于女性,且随时间持续加重。跌倒为首要致病因素,其次是交通伤害与人际暴力。预测2022—2040年中国老年人mTBI年龄标化率将先升后降,男性负担仍高于女性。结论1990-2021年,全球老年人mTBI的疾病负担呈改善趋势,而中国呈加重趋势,表明我国面临更严峻的公共卫生挑战。男性为高风险人群,跌倒为主要致伤因素。未来疾病负担在短期上升后将逐步下降,需针对高风险人群与主要致伤机制实施精准干预。
