摘要(Abstract):
目的探讨超声引导下颈椎间盘射频热凝术(PCDRFT)联合背根节脉冲射频(DRG-PRF)治疗颈椎间盘突出症(CDH)的临床疗效与安全性。方法选取2024年9月至2025年12月收治的CDH患者50例,随机分为联合治疗组(CP组,n=25)与单纯DRG-PRF组(P组,n=25)。比较两组术前及术后1天、1周、1月、3月、6月的视觉模拟评分(VAS)、颈椎功能障碍指数(NDI),并记录手术时间、CT扫描次数、辐射剂量及并发症。结果CP组术后各时间点VAS评分及NDI改善率均显著优于P组(P<0.05)。CP组手术时间较P组长[(45.2±8.6)minvs(32.4±7.2)min,P<0.01)],但其CT扫描次数[(1.2±0.4)次vs(2.8±0.6)次)]及辐射剂量[(186.5±42.3)mGy·cm²vs(354.7±58.1)mGy·cm²)]均显著低于P组(P<0.01)。两组均未发生严重并发症。结论超声引导下PCDRFT 联合DRG-PRF治疗CDH,能更有效地缓解疼痛、改善功能,其精准引导特性有助于显著减少辐射暴露,且安全性良好,值得临床推广。
