摘要(Abstract):
肺康复(PR)是慢性阻塞性肺疾病(COPD)管理的核心,但在干预时机、长期维持及服务模式上仍存挑战。本文综述了PR 在COPD各阶段的实施进展。研究表明,将PR前移至早期(轻中度)患者可显著获益并延缓疾病进程;在急性加重期(AECOPD),早期介入能降低再入院与死亡率,但需严格评估患者运动耐受与安全风险。针对PR获益易衰退的难题,结构化随访与远程康复展现出良好的维持潜力。此外,远程PR极大提高了服务可及性,特别是结合动作观察(AO)和运动想象(MI)的认知远程康复(CtR),为无法耐受传统运动的重症高龄患者提供了创新替代方案。未来应聚焦全病程个性化PR路径的优化及数字医疗的临床价值验证。
